Associates working 24 hours per week or more are eligible
for:
A PHO plan is offered, including a prescription drug
plan. Coverage is available for eligible dependents.
A PPO or DHMO is offered through Cigna Dental. Coverage
is available for eligible dependents.
You and your eligible dependents are automatically enrolled
in a Vision discount plan at no cost when you select medical
coverage. As an alternative, you may elect to enroll at
an additional cost, in a vision plan offered by VisionCare.
Coverage is available for eligible dependents.
DSI makes an automatic contribution to your 401k account
in an amount equal to 2% of your earnings. You can elect
to defer up to 75% of your earnings into a tax-deferred
account and receive additional matching funds from DSI equal
to 50% of the first 4% of your contribution. You have the
option to direct your funds into a variety of investment
options. The plan has a three year vesting schedule.
These plans offer salary replacement for a portion of your
earnings should you become disabled. They are provided for
you at no cost.
These plans offer coverage equal to twice your annual base
pay. They are provided for you at no cost.
Additional coverage is available for purchase through payroll
deduction at group rates for yourself and eligible dependents.
These offer the flexibility to defer money on a pre-tax
basis for future expenses.
Education assistance is available for a variety of healthcare-related
careers, some at 100% regardless of length of service.
